Book - Jazz And Death

Anyone have this book? Comments? I've given it a few browses at the bookstore, and got a few surprises. Like I knew about Benny Goodman's back troubles and pacemaker, but didn't know he had a colostomy from the sixties on. General comment from browsing: seems a lot of jazz musicians have had a yen for self-destruction, and just plain bad luck.

The author is elderly and declined our invitation to speak at our convention (International Association of Jazz Record Collectors) this year.
He published only about 1/3 of his manuscript due to size constraints.
